Specialist Community Public Health Nurse - School Nurse

An exciting opportunity has arisen for a Specialist Community Public Health Nurse post within the South Derbyshire and South Dales locality.
You will work as a clinical leader within the 5-19 service as part of the wider 0-19 Children's Services team within Derbyshire Community Health Services NHS Foundation Trust, delivering the healthy child programme to children, young people and families as part of a Section 75 contract.
You will be responsible for a defined caseload of children and young people, working closely with the schools and multi-agency partners to improve public health outcomes.
You will also be part of the multi-disciplinary team including the wider local children's workforce, working within a public health framework to meet the prioritised health needs of children, young people, and families.
(2024/25 pay award is due to be reflected in pay from October 2024, with any arrears due paid in November 2024)
Main duties of the job
1. To work as a clinical leader within the 5-19 service.
2. To have a defined caseload of children and young people, working closely with schools/multi agency partners to improve public health outcomes.
3. To participate in the identification and safe management of vulnerable children and safeguarding families.
4. To achieve equity of health outcomes for the local population through the provision of an evidence based and needs led service.
5. Provide targeted interventions for children and young people who meet the referral criteria with emerging needs.

Person Specification
Education and Qualifications
* First Level Registered Nurse
* Specialist Community Public Health Nursing qualification (School Nurse)
* Evidence of recent post registration learning
* Recognised teaching and assessing qualification
Experience and Knowledge
* To have actively practised as a School Nurse within the last 5 years or recently undertaken SCPHN training
* Sound understanding and awareness of community and public health agenda
* Demonstrates a holistic approach to the family/young person
* Understanding of the extended children's services including integrated team working
Skills and Attributes
* Excellent interpersonal and communication skills
* Team Building - team working
* Clinical Leadership
* Ability to influence and motivate
* Ability to develop specialist health interest and to share knowledge.
* Able to travel across the whole of the county as needed primarily to attend meetings and training
* Occasional flexibility required including evening and weekend work to meet the needs of delivering the public health agenda
* Willing to train and under PGD provide Emergency Hormone Contraception in outreach venues.
* Experience of working with electronic patient records (SystmOne or equivalent)
* Knowledge and understanding of using social media in a professional capacity
* Basic knowledge of IT programmes, for example, Outlook, Word, Excel, PowerPoint.
