Hatched Recruitment is working closely with the local authorities to find two case managers for our take home and settle service, which is Islington's 7-day rapid service assisting individuals from hospital to community settlement. Candidates must have experience in hospital discharge work, completing Care Act Assessments, and arranging urgent care.

The post holder will have previous experience working with patients and collaborating with other agencies. They will be a proactive, 'can do' individual committed to the model of care and its underlying values. The post holder must demonstrate compassion and understanding for patients, working collaboratively to achieve holistic outcomes.
The post holder will be required to show experience and confidence (or a willingness to be trained to an agreed level of expertise) in communicating with a diverse range of patients. They should have knowledge of care services and how to work effectively in partnership across these agencies. The post holder will work reflectively and be open to sharing experiences with others to demonstrate improved practice over time.
Duties/Functions
The Case Manager role supports the health, social care, and mental health needs of older people with higher-level needs in the West London area through the implementation of Whole Systems. This approach integrates primary healthcare, mental health care, social care, self-care, voluntary sector care, and specialist needs under one umbrella.
The post holder is a key team member whose overarching goal is to work collaboratively to provide improved and more effective care. For patients at high risk of hospital admission, case management addresses all of a patient's needs, as identified in a care plan, with the assistance of a named primary care contact.
* Knowledge, Training and Experience
* Knowledge and understanding of the issues facing individuals who may need care and support.
* Previous experience in a similar position within the public or health sector.
* Understanding of the basic concepts of the Care Act, Mental Capacity Act, and the protection of adults at risk of abuse.
* Experience of carrying out basic clinical duties (relevant experience from a healthcare setting is essential).
