Job overview
Specialist Occupational Therapist Band 6
We have an exciting opportunity for a Specialist Occupational Therapist to join the Community Therapies service at Furness General Hospital.
The Discharge To Assess Team assesses patients in their own homes at the point of discharge. The post is based at Furness General Hospital and is currently part of our Cross Bay Discharge to Assess team covering discharges to the Lancashire, South Lakes and Furness area.
This well-established service is going through a period of transformation to provide a 7-day service to provide holistic assessments to patients at the point of discharge from hospital. This includes identifying support needs, rehab needs and making referrals to other appropriate health and social care services.
Main duties of the job
As a confident clinician with experience working with a wide range of clinical presentations, you will complete a dynamic and holistic assessment with the patient and/or family at the point of discharge from hospital.
Integrating assessment findings, information from several sources and using dynamic and positive risk-taking principles, you will support the patient/family to overcome risks, agree initial support at home and make ongoing rehab and social care referrals. You will need to be prepared to work in complex and evolving situations and be confident in clinical decision-making and reasoning.
Requirements
