The registered nurse, Disability Health and Clinical Advisor focuses on the provision of disability-related health services under the National Disability Insurance Scheme (NDIS) and quality and service improvement activities across the organisation.
The registered nurse, Disability Health and Clinical Advisor reporting directly to the Director, Disability Health, will work in a team of registered nurses and collaborate with other health professionals to provide clinical advice and support to Yooralla clients and their support circle. The role also encompasses the provision of training, undertaking audits and reviews, contributing to the development of policies, procedures and processes that ensure evidence-based practice remains contemporaneous within the organisation.
The registered nurse, Disability Health and Clinical Advisor leads and supports complex clinical care processes alongside the operational division service managers and senior managers to establish and promote the delivery of high-quality health and wellbeing supports to clients in the manner they choose.

Key Responsibilities
Specific Role Responsibilities
* Completing health assessments and developing health support plans
* Delivering individualised health specific training to support workers and education to clients
* Implementing registered nurse delegation and supervision of care as appropriate
* Complete clinical documentation and billing in a timely manner in accordance with Yooralla’s policy requirements
* Provide clinical advice and support to clients and the wider organisation on health related matters and areas for service improvement
* Provide clinical advice on infection control and management / prevention of infectious diseases
* Conduct assessments of direct support workers’ competencies relevant to health supports
* Inform and contribute to the development of Policy and Procedures
* Provide information / reports to Board and sub-board committees as required
* Review incidents related to health and provide clinical support to direct support workers and managers
* Provide advice and support for hospital discharges where complex health supports may be ongoing
* Monitoring and reporting on health support risks and incidents
* Conduct service audits specific to health and clinical practice
* Identify opportunities for improvement to systems, processes and work practices
* Use practice frameworks that ensure the involvement at each decision-making point for clients, families and significant others.
* Promote and facilitate access to health care for clients to achieve positive health outcomes
* Where supports are identified as “high-risk’, take initiative to ensure that appropriate assessments are conducted and appropriate supports are implemented to ensure safety for clients and staff.

* Working to ensure high-quality and safe supports and services for NDIS participants, meeting and exceeding the NDIS Practice Standards and Quality Indicators.
* Meeting obligations under all relevant Victorian and Commonwealth legislation.
* Working in accordance with the Yooralla Code of Conduct and Employee Manual.
Selection Criteria
Key Knowledge Areas
* Current health / medical / disability knowledge.
* Knowledge of the Australian health system and the ability to apply this to practice within the disability sector.
* Postgraduate qualifications in health (desirable).
* Qualification in vocational education or equivalent (desirable).
* Excellent understanding of implementing preventative health strategies in a person-centred manner.
Mandatory Requirements
* Bachelor of Nursing or recognised equivalent.
* International Police Check (if required).
* NDIS Worker Screening Check
* Current proof of ID in Australia
* Current AHPRA Registration
Professional Experience
* A minimum of 5 years of experience working as a registered nurse.
* Working with people with disabilities and their support networks (desirable).
* Experience in clinical assessments and care plan implementation.
* Experience in working under NDIS Practice Standards and implementation requirements (desirable).
* Experience in working in a multi-disciplinary team.
