Clinical Support Worker - Community - Windermere and Ambleside

We are looking for an enthusiastic, flexible and highly motivated Clinical Support Worker to join our friendly and supportive community nursing teams in Windermere, Ambleside and Grange District Nursing teams.
1 x 37.5 hour post
**Please note this role will be working across both teams - Windermere & Ambleside District Nurses and Grange District Nurses**
You will demonstrate passion and enthusiasm for holistic patient centred delivery of care within the community, with a focus on personal and professional development. You will work as part of a dynamic team who give equal importance to staff health and wellbeing, in addition to providing high standard care to our community. The successful candidate will have the opportunity to develop a wide range of clinical skills and knowledge relating to the increasing complexity of care delivered within the community, including wound, pressure ulcer and end of life care.
You will need access to your own transport and the ability to travel as required.
Main duties of the job
This role requires excellent communication skills and it is essential that you have the ability to work on your own initiative and can work independently. The department is committed to support continuous education and professional development to facilitate advancement in skills such as medication administration and catheter maintenance care.
You will work as part of the Integrated Care Community (ICC's) covering a 24 hour period including weekends and bank holidays, 7 days a week. You may also be expected to work across a variety of DN teams in the Community Care Group to maintain service delivery.
Community Nurses work closely with all health and social care disciplines to prevent hospital admission, facilitate hospital discharge and promote independence through reablement and management of long term conditions.
Job responsibilities
Your role will involve working independently and as part of a team to provide support to the Nursing staff and you will be part of a highly dedicated multi-disciplinary team with a focus to provide high, safe quality care for patients in their own homes.
Person Specification
QUALIFICATION
* Care Certificate or the willingness and ability to undertake
* Willingness to undertake NVQ Level 3 or appropriate qualification
* NVQ level 3 or equivalent
Experience
* Experience in a health or social care setting.
* Previous experience of working in a multidisciplinary team
* Experience of working with older persons.
* Experience of working with palliative care patients
Knowledge
* Evidence of recent/ongoing training
Skills
* Ability to communicate effectively both verbally and in writing
* Access to own vehicle/Ability to travel as required
* Basic IT skills
* Venepuncture
* Wound care experience
