Rochford District Primary Care Network (PCN) has a fantastic opportunity for a permanent Data and Outreach Coordinator to join our team. This position is offered on a full-time basis, working 37.5 hours per week, ideally Monday to Friday, and is covering the following practices:
* Ashingdon Medical Centre
* The Practice Leecon Way
* The Puzey Family Practice
* Wakering Medical Centre
The role involves providing high quality, comprehensive, and effective administration skills while providing a central coordination role for patient care planning across member practices.

The successful applicant will have the ability to work without supervision using their own initiative, organizing and prioritizing their own and others' workloads to changing and often tight deadlines.
