Band 6 - Community Psychiatric Nurse, Crisis Resolution and Home Treatment Team (CRHTT)
We are looking for experienced mental health nurse (or social worker) to join our friendly and supportive Crisis Resolution and Home Treatment Team (CRHTT) based in Ware, South East Hertfordshire.

Main duties of the job
You will be:

The role:
7. Providing urgent assessments to those considered to have acute mental health concerns and to robust home treatment options as an alternative to hospital admission.
8. A role that is varied and challenging, offering an opportunity to work in a variety of settings, with people with a variety of mental health needs.
9. As gatekeepers for hospital admission, you will work as part of a multi-disciplinary team to provide safe and effective treatment in the community, and facilitate early discharges from the acute inpatient units.
10. Working collaboratively with our service users, their carers and our colleagues to positively manage risk in the acute care pathway, whilst enabling an individuals' recovery journey.

Job responsibilities
19. You will support the Team Leader in the development and implementation of a multi professional Crisis Resolution/Home Treatment Team providing a high quality service to meet the needs of people in acute phases of mental illness, and their carers.
20. To ensure efficient and effective running of the team, providing Leadership and supervision within the multi-disciplinary staff group as appropriate.
21. You will undertake urgent assessments of people with mental health problems in acute crisis and where appropriate provide intensive community based treatment/support services as an alternative to inpatient admission.
22. The service will be available 24 hours a day, 365 days a year.
23. You will be responsible for helping to establish and maintain team systems and processes for effective team working and processes, including outcome measures and audit under the direction of the Team Manager

Experience
Essential
24. Post-registration experience with at least some experience at band 6
25. Experience in acute and community areas.
26. Evidence of continuing professional development.
27. Experience in care planning, change management and use of Microsoft Office
28. Knowledge of physical health contributions to Mental Health issues, difficulties and assessment
Desirable
29. Experience in working with acute mental health problems in community setting
Qualifications
Essential
30. RMN or Dip / BSc (Hons) in Mental Health Nursing
Desirable
31. ENB 998 / Preceptorship and Mentorship.
32. ENB 812 / BSc (Hons) Specialist Community Nursing Practice (Mental Health) or similar training.
