Older Adults Mental Health Practitioner (RNMH or OT)

Derby City Older Adult Community Mental Health Team (CMHT) & the Derby Wellbeing Team are looking for an enthusiastic Band 6 Older Adult Specialist to join their friendly and supportive team based in Derby City.
You need to have experience of working with older adults with mental health conditions, in a community setting.
This newly-developed role, hosted by the Older Adults CMHT, will involve supporting introductions into the Derby Wellbeing team, identifying the needs of older adults with mental health conditions who live within the local community, and designing & implementing person-centred support to enable people to live their best life. This will include working with colleagues across the local community to ensure that we provide the right service at the right time by the right person.
The role is supported by the Derby City Older Adults CMHT, but working into the newly developed Derby Wellbeing Team.
Derby Wellbeing is a locality-based, multi-agency team, offering seamless support from agencies by placing the person at the centre of their support needs.
This role involves supporting the development of the offer for older adults through prototyping.
A core aspect of the framework is supporting those who may not have been eligible for secondary mental health support previously.
You will have the ability to travel across the geographical area.
Main duties of the job
The post holder will be responsible for supporting the delivery of a responsive local service, taking forward an Integrated Primary Care Wellbeing Pathway in the Derby City area.
A key feature is to support developing the Living Well offer for Older Adults to ensure this population can take advantage of this approach.
As outlined in the Community Mental Health Framework, the offer is for people experiencing functional mental health conditions.
It is essential to have knowledge of organic conditions, ensuring people receive support from appropriate services.
Learning objectives include how working into the Derby Wellbeing team as an older adult practitioner can be most effective, and how Derby Wellbeing can be an extension of the Older Adult CMHT. This includes developing links with community organisations.
The role involves undertaking person-centred psychosocial assessments (conversations), safety planning & formulation.
You will support individuals to improve their mental health by helping develop self-care skills, build resilience, and utilise community resources.

* Professional registration with relevant body, eg NMC (RNMH), HCPC
* Member of RCN/RCOT
Experience

* Experience working with people who have functional conditions and those who have organic conditions
