An exciting opportunity has arisen for a Band 5 Community Nurse based in Haverfordwest.
The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.
The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patients/carers, and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.
Pembrokeshire's Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.
This post is worked over a 7-day period including weekends and bank holidays. The post holder will need the ability to travel between sites in a timely manner.
Be flexible and adaptable in order to accommodate the needs of the service, be able to contribute to the performance of the community nursing team and develop professionally within the role.
Assessment of nursing needs, deciding on the use of appropriate assessment tools, to plan appropriate nursing care, deciding on frequency of visits, deciding when care should be reviewed, altering care plans accordingly, in conjunction with the District Nurse Team Leader. This will ensure successful outcomes for appropriate care designed and adapted to meet individual needs. All care provision is based on robust evidence-based clinical research.
Work alone and with confidence to make decisions without supervision. Work in line with the Health Board Lone Worker policy.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
