The West Kent Therapy Service is looking to recruit an enthusiastic and talented Band 6 Occupational Therapist to join our established Community Rehabilitation and MDTTherapy Team.This post is part of the dynamic service based in Tonbridge made up of Occupational Therapists, Physiotherapists, Apprentices, and Therapy Assistants. We providecomprehensive assessments, rehabilitation and moving and handling solutions for patients in their own homes in rural and urban areas across the locality. Ensuring they achievetheir full potential following a hospital admission or period of illness, and to keep those living in their own homes safe with the greatest level of functional independence possible.The service responds to the rehabilitation needs of adults of all ages and backgrounds with a wide range of problems including, elderly care, complex co-morbidities, neurological,and orthopaedic.This role will also the support the frailty clinics in Edenbridge, which are a multi-disciplinary project targeting the population more proactively to promote ageing well. Bringingthe therapy voice to how we can support this cohort, with advice, education and treatment solutions.
Main duties of the job
o Domiciliary and clinic based interventiono Completing comprehensive and holistic new patient assessmentso Designing individual rehabilitation plans and goal setting.o Delivering and adapting Occupational Therapy interventions.o Collaboration with MDTo Equipment provision and orderingo Supervising Therapy Assistants and Apprenticeso Clinical education of students on placement

Qualifications
* OT degree and HCPC registration
* QI and practice educator training
Experience
* Minimum 2 year in practice / Experience managing complex caseload / supporting and mentoring others
* Community experience / IT relevant to role
