The role is to support Band 6 nurses in the Dudley Special schools and the Community Children's nursing team. The team works as part of the Community Children's Nurse Team (CCN Team) at Brierley Hill Health and Social Care Centre. The nurse will support in managing the health needs of children and young people with complex health and profound learning disabilities.
Main Duties of the Job
The nurse will:
1. Support in managing the health needs of children and young people with complex health and profound learning disabilities.
2. Work closely with the MDT to ensure that Education, Health and Care Plans are implemented. Some home visits and other community visits will be carried out as part of a holistic case management.
3. Provide clinical skills and ensure a holistic, family-centered approach to assessing, planning, implementing, and evaluating the health needs of the children.
4. Teach/train and support the team as well as the school staff to ensure the health needs of the children are met.
5. Implement strategies to support the care of children with learning disabilities.
6. Manage emergency health situations in the school.

Job Responsibilities
Main Duties/Responsibilities
1. Demonstrate empathetic communication skills when dealing with patients and carers along the patient pathway.
2. Provide specialist nursing advice and support to families of children with significant medical needs under the direction of the band 6 in the special schools.
3. Complete home visits as required supporting the families and children and assess the child's clinical condition.
4. Undertake specific clinical tasks and skills for which the nurse has received training and is deemed competent in all clinical settings, including suctioning, oxygen management, tracheostomy care, care of central lines and vascuports, and other clinical procedures.
5. Encourage children and families to participate in their care by providing training and support on a range of clinical procedures to meet the health needs of the child within scope of expertise.
6. Work in conjunction with the band 6 and other agencies to implement agreed transition of care processes ensuring it takes place in a timely fashion and meets the ongoing needs of the child/young person.
7. Have knowledge of the SEND agenda and EHCPs and contribute to EHCPs and the reviews to ensure that the optimum support and care is provided for the child and family.
8. Clinically assess the child or young person and ensure that actions are taken to promote safety.
9. Support the safe administration of medications within the school and support staff to be trained adequately and that their competencies are assessed.
10. Provide and maintain a high standard of skilled nursing care/skills for children with additional health needs.
11. Facilitate care at home for children with short term/complex health conditions including children who are palliative.
12. Provide reports, as appropriate for CYPs EHCP reviews.
13. Demonstrate knowledge, skills, competency, and innovative practice in paediatric nursing care.
14. Demonstrate continual evaluation of clinical practice and recommend changes.
15. Assist in the completion of detailed holistic assessments of children and families, while giving consideration to cultural differences.
16. Ensure that the changing needs of individual children and families are identified and that adjustments are made to the child's care plan.
17. Establish or maintain relationships with the child and family where relevant whilst the child is an inpatient in hospital to help facilitate smooth discharge.
18. Complete accurate and timely recordings of involvement with the child and family and report any changes affecting the child and family appropriately.
19. Support in the development of integrated care pathways.
20. Work in conjunction with other nurses in the team and provide cover as required depending on scope of practice.
Managerial Responsibilities
1. Maintain an electronic portfolio of evidence that offers assurance against CQC essential standards and revalidation.
2. Line manage unregistered staff in the school if required - supported by band 6.
3. If required, take responsibility for the management of the school in the absence of the band 6.
4. Work under the overall direction of the Band 6, but maintain accountability and responsibility for own work.
5. Inform line managers of any issues affecting safe and effective service delivery at the time an issue is highlighted.
6. Actively demonstrate commitment to clinical supervision.
7. Participate in the development of policies, pathways, and clinical guidelines.
8. Demonstrate the ability to work autonomously and manage time effectively.
9. Be aware of budgetary considerations in line with the trust financial plan.
10. Ensure that any equipment provided by the Team is regularly serviced and maintained, and that families are using the equipment correctly and safely.
11. Coordinate home visits alongside other team members to ensure time effectiveness and increase availability of community nursing services for children and families.
12. Demonstrate an ability to work across organisational and professional boundaries as directed by the line manager.
Educational/Training Responsibilities
1. Under the direction of the band 6, provide training for the schools about complex medical needs/disabilities and the support services available for these children.
2. Facilitate training and implement teaching plans for parents and school staff to empower them to manage a child/young person's condition.
3. Assess the competencies of school staff and delegate care tasks as appropriate to education staff.
4. Maintain knowledge and competence in computer skills, in order to access drives, databases, email, and approved internet resources and critically appraise sources of information and their evidence base.
5. Maintain clinical skills and practice in order to offer support and direction to clinical teams and other multi-disciplinary teams.
6. Maintain professional registration and revalidation.
7. Review clinical and practice standards against guidelines produced by regulatory bodies.
8. Participate in training needs analysis.
9. Act as a clinical resource.
Research Responsibilities
Demonstrate knowledge of clinical trials relevant to specialist area. Identify areas of research/audit with the team, sources of funding, and in conjunction with the Team Leader, initiate research and audit projects.
