We aim to support delivery of highquality, cost effective holistic care to the population we serve, through thesharing of best practice, and by ensuring an equitable approach to enableeffective team partnershave strong links with the local VTS, the ICB including representation on thelocal Prescribing Committee, and with secondary care.
Our team currentlyconsists of 5 GP Partners, 2 Salaried GPs, 2 nurses, HCA, TNA ,2 pharmacist, PracticeManager, Deputy Practice Manager, Operational Lead, 5 admin and receptionistteam, 2 medical secretaries, a mental health link worker, a social prescribingnavigator,a supportive admin and management team.
Main duties of the job
The role will involve working independently and within a highly motivated additional roles team to support further development of our prescribing systems and pathways. Working to ensure safe, effective and efficient prescribing within practice.

Bute House Medical Centre Pharmacy Technician
Pharmacy technicians play an importantrole, complementing clinical pharmacists and community pharmacists.
The purpose of the Pharmacy Technician Role is tolead improvements which maximise safe, cost-effective best practice inprescribing to improve the quality of patient care. The post holder will supportpatients to get the best from their medicines by switching medications toagreed and approved protocols, improving repeat prescribing and drug monitoringprocesses in General Practice, including promotion of repeat dispensing andonline ordering, minimising clinical risk and aiming to reduce wastedmedicines. In addition, the post holder will be responsible for encouraging thedevelopment of better understanding of the principles of medicines optimisationthroughout the practice teams and promoting good practice in line withtherapeutic developments. This will involve assisting practice inachieving national requirements, NICE implementation and utilisation ofmedicines optimisation initiative.
KeyResponsibilities

Clinical
Undertake patient facing and patient supportingroles to ensure effective medicines use, through shared decision-makingconversations with patients.
Carry out medicines optimisation tasks includingeffective medicine administration ( checking inhaler technique), supportingmedication reviews and medicines reconciliation.
Where required, utilise consultation skills to workin partnership with patients to ensure they use their medicines effectively.
Monitor high-risk medicines according to local systemsand processes
Supportmedication reviews and medicines reconciliation for new care home patients andsynchronising medicines for patient transfers between care settings, linkingwith local community pharmacies
Supportdelivery of Structure Medication Reviews
Provide expertise to address both the public healthand social care needs of patients, including lifestyle advice, serviceinformation, and help in tackling local health inequalities. Manage shared careprotocols and liaise with Clinical Pharmacists and other MDT members for morecomplex patients. Support initiatives for antimicrobial stewardship to reduceinappropriate antibiotic prescribing locally.
Technical and administrative
Implement policies and effective processes formedicines governance including controlled drugs and repeat prescribing and drugmonitoring.
Ensure all information, including allergy, isrecorded accurately and contemporaneously
Identify missing information, discrepancies anderrors on repeat prescription requests and resolve according to local policyand procedures.
Support the team to ensureefficient medicines optimisation processes are being followed.
Implement efficient ordering and return processesand reducing medication wastage.
Provide training and support on the legal, safe andsecure handling of medicines, including the implementation of the ElectronicPrescription Service (EPS).
Promotion of Electronic Repeat Dispensing (eRD) andonline ordering
Develop relationships with other pharmacyprofessionals and members of the multidisciplinary team to support integrationacross health and social care including primary care, community pharmacy,secondary care and mental health.
Supportpractice reception teams in streaming general prescription requests, so as toallow GPs and clinical pharmacists to review the more clinically complexrequests.
Support the implementation of national prescribingpolicies and guidance within GP practice, care homes and other primary caresettings.
Support the practice to deliver on QIPP agenda, QOF andlocally commissioned enhanced services Support the practice in reviewing anddeveloping practice policies for CQC requirements.

1. Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy
