This is a part-time position of 34 hours per week over 4-5 days. The Cardiac Rehabilitation Service provides care, support, education and exercise options following a cardiac event such as MI, PCI and cardiac surgery. You will work alongside the multi disciplinary team to help provide this. You will ideally have some cardiac nursing experience, however, full training and support will be provided. We would encourage an informal discussion about the role. Main duties of the job The service operates Monday to Friday, we do not work weekends or Bank Holidays. This is a Trust-wide role ,with bases at Normanton Health Centre and Dewsbury District Hospital. You will be expected to work across hospital and community sites as per service need. You will be responsible for actioning/processing referrals received into the cardiac rehabilitation service as appropriate. Providing inpatient cardiac rehab advice to eligible patients within the hospital setting. Contacting patients and providing immediate post discharge advice following their cardiac event and arranging appropriate appointments on behalf of the Clinical Nurse Specialist. Deal with enquires, offering advice and support to patients. Work alongside the MDT at the Cardiac Rehabilitation exercise programmes and deliver education sessions. Conduct final assessments for patients at the end of their cardiac rehabilitation journey/programme You will be working and liaising with the multidisciplinary team i.e. Physiotherapists, GP's, Cardiologists etc. Maintaining data input on numerous applications e.g. System One, Solus, Microsoft excel.
