The Community Learning Disability Service provides integrated specialist health and social care to adults with learning disabilities in Tower Hamlets. Additionally, the service is responsible for the provision of a specialist day activity hub - CREATE.
An exciting vacancy opportunity has arisen for a Band 7 specialist nurse within our Mental health and Behavioural Support team. This team focuses upon supporting people with learning disabilities who have enduring mental health problems and/or require specialist interventions around behaviours that others find challenging.
Main duties of the job
You will be expected to:
1. Use advanced specialist nursing skills in working with people with learning disabilities to maintain, promote and improve health.
2. Support people with learning disabilities to develop their full potential by reducing the impact of their impairment that could limit their social inclusion in a person-centred approach to promote choice, independence and inclusion.
3. Facilitate access to health care services through collaborative and partnership working in developing shared care with primary and secondary health services e.g., GP Link Role, Nurse Lead Epilepsy Clinic, and Sexual Relationship & Health Promotion etc.
4. Contribute to shaping local health services (e.g., development of local protocols and policies), promoting an inclusive service for people with learning disabilities.
5. Undertake assessments of individual's potential eligibility for NHS Continuing Healthcare Funding in accordance with the National Framework for NHS Continuing Healthcare.
6. Have substantial knowledge of the health and social needs of individuals with learning disabilities: awareness of current legislation and best practice in the field of learning disability nursing.
7. Work collaboratively with the Lead Nurse to provide a high standard of clinical leadership to staff within the Community Nursing Team to ensure effective multi-disciplinary working.
8. Lead on transforming care and offer consultation to inpatient wards.

Person Specification
Education/ Qualification/ Training
Essential
* Registered Mental Health Nurse or LD nurse
* Evidence of continual professional development
* Current Professional Registration
* Recognised post graduate qualifications in mental health or Learning disability nursing
* ENB 998 or equivalent
Experience
Essential
* Substantial post qualification experience in the field of older adult mental health/dementia/learning disabilities in community settings.
* Previous experience at working autonomously
* Experience of joint working with ASC/CHS/GP's and others.
* Experience of working in a multidisciplinary mental health team
* Experience of working in a multi-cultural environment and of ensuring that nursing practice is culturally sensitive
* Essential and demonstrable experience of writing concise and clear clinical reports
* Extensive and demonstrable experience of assessing, planning, implementing and evaluating programs of care, particularly in an older adult community setting
* Experience of developing skills, knowledge and attitudes in staff.
* Experience of delivering teaching
* Experience of developing or contributing to the development of new services preferably in mental health

* Demonstrate an understanding of how to promote the rights, responsibilities and recovery of adults with LD with mental health problems
* Demonstrate a full understanding of the key issues and priorities relating to the provision of mental health services in integrated care
* Demonstrate a clear understanding of current legislation and policy pertaining to the provision of mental health services in all settings including the Mental Health Act 1983, Mental Capacity Act 2005, NHS and Community Care Act 1990, Human Rights Act 1998
* Demonstrate a thorough knowledge, expertise and practical experience of a full range of therapeutic nursing skills and approaches to people with mental health problems aged 16 years and above
* Demonstrate an awareness and understanding of child and adult protection issues
* Demonstrate a clear understanding of clinical governance and the provision of evidenced based care
* Demonstrate a clear understanding and commitment to implementing the Equal Opportunities policy
* Demonstrate well developed and confident professional nursing skills
