HASP PCN is looking to recruit a part-time (16 hours) Mental Health Support Worker SPLW who will specialise in providing non-clinical support to our patients with low-level mental health concerns. Ideally, you will have previous experience of working with complex or vulnerable people and be able to empathise with their situation, intervene, and signpost to the appropriate services.
Main duties of the job
Working under the direct supervision of the Strategic Manager, provide social prescribing that empowers people to take control of their mental health and wellbeing and shift the focus back to what matters to them.
You will work as a key part of the primary care network (PCN) multi-disciplinary team, helping the PCN to strengthen community and personal resilience and reduce health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing, and physical inactivity, by increasing people's active involvement with their local communities.
Job responsibilities
1. Take referrals from HASP member practices.
2. Gather information, triage accordingly, and record details on clinical system.
3. Quickly build rapport and provide personalised support to individuals, their families, and carers to take control of their health and wellbeing, live independently, and improve their health outcomes.
4. Develop trusting relationships by giving people time and focusing on what matters to them.
5. Take a holistic approach, based on the person's priorities and the wider determinants of health.
6. Meet people on a one-to-one basis, by telephone, face to face, or by making home visits where appropriate.
7. Recognise emergency situations and act accordingly in line with training and local safeguarding policies.
8. Manage and prioritise your own caseload, in accordance with the needs and priorities of individuals.
9. Work closely with the voluntary and community sector to draw on and help to increase the strengths and capacities of local communities.
10. Champion Social Prescribing and support educating non-clinical and clinical staff within their PCN multi-disciplinary teams on available community services.
11. Be proactive in developing strong links with local agencies to encourage referrals.
12. Seek advice and support from the GP supervisor and/or identified individuals to discuss patient-related concerns.
13. Work closely with the voluntary and community sector to stay updated on developments within the organisation and the wider local voluntary sector.
14. Adhere to organisational policies and procedures within the PCN, including confidentiality, safeguarding, lone working, information governance, and health and safety.
15. Work with your supervising GP to access regular clinical supervision.
16. Undertake any tasks consistent with the level of the post and the scope of the role.
