Specialist Physiotherapist Long Term Neurological Conditions
We are looking for a motivated and enthusiastic physiotherapist to join our long term neuro team.
The post holder will be part of the North Tees & Hartlepool NHS Therapy Service providing high quality care to patients who are referred for neurological rehabilitation, as an outpatient, either in the Acute Trust or community/ domiciliary settings.
Main Duties of the Job
1. Undertake a team coach role, alongside the Clinical Team Leader in planning, co-ordinating, delivering and evaluating the therapy Service provided to patients who require neurological rehabilitation.
2. Take a lead role in the advanced assessment and treatment of patients within the specialty who may have complex and/or chronic presentations, determining clinical diagnosis and therapy treatment indicated, and maintaining records as an autonomous practitioner.
3. Plan, deliver and evaluate physiotherapy intervention in partnership with MDT colleagues; and together lead the planning, delivery and evaluation of all elements of the programme.
4. Assess and treat patients in a variety of locations including the acute hospital setting, outpatients, community, intermediate care, residential/nursing homes and in the patients' own home.
5. Be responsible for the management of Band 5 Physiotherapists, Specialist Therapy Assistants and students when required.

Job Responsibilities
1. Be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care. Ensure a high standard of clinical care for the patients under your management, and support band 5 staff to do likewise.
2. Interpret and analyse clinical and non-clinical facts to form an accurate diagnosis and prognosis in a wide range of highly complex conditions, recommend the best course of intervention, and develop comprehensive care/therapy plans.
3. Represent the trust externally (locally, regionally and nationally) regarding therapy services provided to patients who require therapy input.
4. Undertake the comprehensive assessment of patients, including those with complex presentations, using investigative and analytical skills, and formulate individualised management and treatment plans, using advanced clinical reasoning, and utilising a wide range of treatment skills and options to formulate a specialised programme of care.
5. Accept clinical responsibility for a designated caseload of patients, and organise this effectively and efficiently with regard to clinical priorities and use of time.
Person Specification
Qualifications and Training
* Physiotherapy Degree
* Experience working with patients with neurological conditions
* Leadership qualification or relevant experience
* Hold a valid UK Driving licence
Experience
* Knowledge and experience of the outpatient management of patients with long term neurological conditions
