The Pathfinder Team is a specialist community team based within North Devon District Hospital, consisting of professionals from nursing, therapy, and paramedic backgrounds. Our remit is to holistically assess and then procure onward care, give advice, issue equipment, and support with therapy needs for specific patient groups, including:
1. Admission avoidance
2. Short stay admissions ward discharges
3. End of life care discharges
4. Frailty Acute Hospital at Home
Main duties of the job

Job responsibilities
Our role can be challenging and time-sensitive in order to support patient flow through the acute hospital, but it is also rewarding and varied. You will have the opportunity to work in many different departments and wards throughout the acute site, liaising with many different teams and specialists to facilitate appropriate discharge requirements for the patients referred to us. We follow patients into the community to provide acute hospital at home care.
We work closely with our community colleagues and have established ourselves as a link between acute and community care to provide a seamless transition of care between hospital and home or other receiving destinations. We are currently developing a Frailty pathway and are involved in many initiatives around length of stay and discharge planning, frequently receiving excellent feedback from our patients and professional colleagues.
We require a forward-thinking, dynamic, motivated, and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will possess. Experience from both acute and community backgrounds is preferred but not essential. Experience and understanding of the Continuing Healthcare Framework, Mental Capacity Act, and Safeguarding would also be an advantage, but training can be provided.

This is a fixed-term contract for 6 months. For current NHS employees, this post could be a secondment opportunity.
