Job summary An exciting opportunity to be part of a dynamic team at the heart of hospital discharge has arisen. The post is for a Band 5 Social Worker working within the Discharge to Assess team covering the South West area. Multi-professional working is a key skill required as you will work alongside therapists, care providers District nurses, Users and carer's Mental Health services and commissioners to support people to return home and regain their independence. Good communication skills are a necessity The interventions are short term and the work is of a fast pace but rewarding when the outcome is that the person is able to live as they choose in their own home. Main duties of the job As a Band 5 Social worker you will undertake the more complex interventions so will require good knowledge of The Mental Capacity Act and The Care Ac to guide practice and ensure the best outcome for users and carers.
