Principle Responsibilities Ensuring a high standard of care with intermediate care bed based facility. Liaises with the Intermediate Care Sister / Charge Nurse to ensure information in relation to patient care/changes to care plan and the continuity of care. Provides mentoring, teaching and assessing to support staff. Liaises with all health care professionals and other agencies to ensure effective service delivery in the intermediate care facility. Managerial To be responsible for work allocations of support staff. To complete monthly supervisions for support staff. To be responsible for organising and prioritising own caseload. Leadership Act as a mentor for pre-registration nursing students. Support the development of the nursing team by teaching and assessing members of the team including nursing assistants. Undertake mandatory in-house training and other training that supports the role. Access clinical supervision. Undertake training that is specific to the post. Teaching of patients / carers and other professionals to support patients to maintain their optimum level of independence Clinical Perform comprehensive assessment of patient nursing needs. Assess, plan, implement and evaluate programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse. Maintain accurate records in accordance with NMC. Attend a multi-disciplinary case conference as the named nurse if appropriate. To communicate and collaborate with other members of the Integrated Intermediate Care Team, PHCT, voluntary and statutory organisations to support individual patient care. To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations. To competently perform clinical skills related to the post.
