Recovery Coordinator Tier 4 Lead

What you will be doing?
You will be responsible for contributing to the targets set by Forward and the local commissioners. One of the key targets will be measuring outcomes, so cooperative working relationships with other partner agencies are vital to this role.
You will manage a caseload of service users who require inpatient treatment. This includes supporting them through the application process to acquire funding, accompanying or representing them at panel, exploring residential facilities to find the best fit for their needs, and ensuring they have completed appropriate preparation prior to inpatient treatment. You will be the main point of contact for residential facilities and will be expected to keep the locally held information about each facility up to date.
Flexibility will be required in this role, which may include some evening/weekend shifts with reasonable notice to deliver services, enabling continuity of care for the client group.
What we are looking for?
We are looking for motivated, self-driven, and passionate individuals to work with our client group and support them on their way to recovery. The skills and experience listed below are desirable but not essential, as we offer a robust induction and ongoing training program to develop our workforce:
1. Knowledge of OST and harm reduction.
2. Experience and motivation to engage individuals through proactive outreach.
3. Experience of engaging and motivating individuals to change.
4. Experience of working within substance misuse services.
5. Experience of carrying out comprehensive assessments, risk management plans, and the design and implementation of SMART care plans.
6. Speaking multiple languages is desirable but not essential.
7. Experience of delivering structured interventions to service users.
8. Experience of facilitating groups.
9. Using motivational interviewing techniques in both 1:1 and group settings.
10. Strong IT skills.
