An exciting opportunity has arisen for a Band 5 Nurse to work as a Mental health Practitioner within the Assertive Outreach (AO) Service. AO is a tertiary service who takes all referrals from Secondary care services. We are looking for an enthusiastic, committed, hardworking individualto join the team. You willwork under the guidance of the Senior Practitioner as a member of the MDT providing community services to adults with a enduring mental health problems in their own home. You will hold a caseload of people with enduring mental Health difficulties and be responsible for the assessment, planning and evaluation of packages of care. You will require excellent communication skills and be able to communicate with a wide range of people both verbally and in written format.
Main duties of the job
The successful candidate will be a Lead professional for a allocated caseload of patients following the principlesof the Care Co-ordination policy, being an integral member of the MDT, attending and contributing to MDT discussions. To maintain accurate record record keeping in line with NMC guidelines and Trust policies and procedures, co-ordinatingand prioritise your workload on a day to day basis
The post holder will act in a professional manner and adhere to the NMC Code of Professional Conduct at all times, maintainingcontinuing professional development. Actively participatingin the supervision and appraisal processes, adheringto Trust policies, procedures and clinical guidelines. Promote patient dignity, equality, diversity and human rights and carry out duties in accordance with the Health and Safety at work Act 1971.
