Job Overview We are seeking a dynamic, experienced and motivated Speech and Language Therapist (SLT) to join our well-established Stroke Early Supported Discharge (ESD) team. This is an exciting opportunity to be part of a multi-disciplinary service delivering high-quality, patient-centred stroke rehabilitation in the community around Frimley Park hospital. As a Band 7 SLT, you will provide specialist assessment and intervention for adults with acquired communication and swallowing disorders post-stroke. You will work closely with patients, their families, and a supportive MDT including physiotherapists, occupational therapists, nurses, psychologist and rehabilitation assistants to enable a seamless transition from hospital to home. Conduct detailed assessments of speech, language, communication and swallowing needs. Develop and implement individualised care plans and contribute to goal-setting within the MDT. Provide clinical supervision and support to junior staff and students. Contribute to service development and quality improvement initiatives. Engage in regular team meetings, case discussions and outcome evaluations. Person specification Qualifications Essential criteria Degree in Speech and Language Therapy Masters qualification/equivalent experience and/or intention of working towards Masters HCPC registration Desirable criteria Relevant further studies to master’s level Skills & Knowledge Essential criteria Excellent oral and written communication skills – both informally and formally via presentations/training. Collaborative team approach to working Able to build effective working relationships, network and assert views, sometime in challenging situations.
