Job Title: Band 6 Senior Community Mental Health Nurse
Salary band: Band 6
Hours: 30 hours per week
Location: Fareham and Gosport OPMH
An exciting opportunity has arisen for a Senior Community Mental Health Nurse to join an established team at Fareham and Gosport OPMH Team.
Responsibilities will include care coordination of service users, Duty Worker on rotation, initial and follow-up assessments, and supervision.

The Community Mental Health Team is focused on delivering a recovery service for clients with severe and enduring mental health issues.
Due to the nature of this post in carrying out home visits and attending patient meetings, a car driver is essential with access to own car. Mileage is paid.

Consideration will be given for the right candidate to be supported in development to Band 6 competencies.
Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community HCSWs where appropriate.
Working under the supervision of the Deputy Team Manager/Team Manager, the practitioner will formulate care and risk management plans in accordance with identified needs and ensure that these are recovery-focused and developed in collaboration with the service user and, where appropriate, their relative and carer.

Person Specification
Qualifications
* Registered Nurse, Mental Health Practitioner.
* Recognised Mentorship qualification.
* Able to attend and participate in all statutory & mandatory training events to enhance continued professional development.
* Willingness to complete further studies as necessary to meet service delivery requirements.
