The Leaving Care Social Worker is responsible for supporting young people aged 16-25 as they transition from the care system to independent living. This role involves working closely with care leavers, helping them navigate challenges in housing, education, employment, and mental health, while ensuring they have the necessary life skills and support networks to thrive in adulthood. Responsibilities include: * Conduct assessments of care leavers' needs and develop personalised Pathway Plans. * Provide practical and emotional support to young people transitioning to independent living. * Help care leavers develop essential life skills (e.g., budgeting, cooking, managing bills). * Support care leavers in accessing housing, education, employment, and health services. * Collaborate with multi-agency teams to ensure coordinated, holistic support. * Advocate on behalf of care leavers to ensure their needs are met by various services. * Ensure compliance with safeguarding protocols and report any concerns promptly. To be successful within this role, you are required to have: 1. Social Work England registration
