To assess, plan, implement and evaluate care. To ensure clear channels of communication exist between clinical, administrative and community teams. To progress the role of the practice nurse in primary health care and contribute to both national and local service development and initiatives. Teamwork: We are seeking a dedicated and collaborative professional to join our multidisciplinary team in a thriving GP practice. This role emphasises the importance of teamwork, requiring you to work closely with GPs, pharmacists, fellow nurses, healthcare assistants, and administrative staff to deliver high-quality, patient-centred care. Your ability to communicate effectively, share expertise, and contribute to a supportive team environment will be vital in ensuring seamless service delivery and positive patient outcomes. Responsibilities: To provide assessment, treatment, screening and health education services and advice, working in accordance to set protocols, guidelines and NMC Code of Conduct Provide nursing service/treatments to patients independently or in participation with General Practitioners with reference to aforesaid agreed protocols and guidelines Professional Responsibilities: To be a Registered General Nurse with current NMC registration To undertake reflective practice and regular performance review in accordance with the NMC recommendations for revalidation of clinical practice To effectively manage own time, workload, and resources, acknowledging limitations to own clinical practice/knowledge and in the delegation to others. Recognising when referral to other health professions is needed Evaluating and adapting care using evidence-based practice in consultation with colleagues as a forum for team education, development and audit Using evidence-based practice to assist in the formulation of practice philosophy, strategy and policies Specific Duties: Nursing procedures to include, but not limited to: Public Health disease prevention, to include adult and childhood immunisation, seasonal and travel vaccination. Chronic disease management. Wound care Immediate and necessary clinical intervention such as venepuncture, ECG monitoring, sample testing. INR monitoring Other Duties: Working in accordance with the NMC Code of Conduct: Concise, accurate record keeping and documentation with competent utilisation of the Vision software Promotion of confidentiality and patient dignity with protection of personal and sensitive data Participation within clinical audit/research and the monitoring of standards of care Participation in the education of students of all disciplines To act as an educational resource for other nurses, health care professionals and non-clinical staff within the multidisciplinary team To adhere to strict Infection Control in line with policies and guidelines (ANTT) Maintenance of treatment room supplies with specific reference to maintenance of the cold chain and stock levels of vaccines. Maintenance and checking of stock equipment such as resuscitation equipment and emergency medication accompanying the equipment Any other duties as reasonably requested by senior staff, inclusive of GPs and Practice Manager, providing those duties are within the scope of the role and that the individual is trained and capable Band 5 General Practice Nurse Qualifications/Recommended Academic Entry Level RGN Level 1 Bachelor of Nursing Degree (BN Hons) SPQ in Practice Nursing Fundamentals of Community Practice IQT training (bronze level) Skills/Tasks Cytology (following CSW training) Wound management Involvement in QAIF Involvement in Supplementary Services (Previously called Enhanced Services) Childhood Immunisation (following training, if required) Administration of other vaccines according to immunisation schedule Risk assessment, travel advice & immunisation (following training, if required) Contraception reviews (following training, if required) Delivery of Depo-Provera/Sayana Press Administration of gonadorelin therapy and other injectable therapies under a PGD/PSD Sexual health advice management of HV swabs and signposting to appropriate clinics Sexual health advice management of HV swabs and signposting to appropriate clinics Administration of B12 injections Infection control champion Waste stream management champion Health Promotion and signposting to screening programmes Referral of patients to other HCPs with a good level of understanding of the principles of accountability and delegation. Management of long-term health conditions, including COPD, Asthma, hypertension, Diabetes Supervision and delegation of tasks to HCSW Communicate effectively with patients and colleagues in a manner which demonstrates a comprehensive awareness and understanding of the NMC code of conduct Band 6 General Practice Nurse with extended skills Qualifications/Recommended Academic Entry Level Bachelor of Nursing Degree (BN Hons) Diploma Modules in CDM (e.g., respiratory, diabetes, minor illness, sexual health) Skills/Tasks Skills/Task of Nurse Band 5 Plus, one or more of the following.
