Winchester City Primary Care Network is seeking an enthusiastic and experienced Pharmacist Service Manager to play a key role in the successful delivery of the Pharmacy Service.
Due to the rapid and positive development of the pharmacy service, we plan to expand and add to our well-established team of Independent Prescribing Pharmacists, Clinical Pharmacists, and Pharmacy Technicians by recruiting a Clinical Pharmacist Service Manager.
The Clinical Pharmacist Service Manager for Winchester City Primary Care Network (PCN) will work as part of the multi-disciplinary general practice team in a role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
The post holder will be a prescriber or will be completing training to become a prescriber and will work with and alongside the general practice team.
The post holder will take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD, asthma, depression) and people with learning disabilities or autism to implement the structured medication review and optimisation set out in the Primary Care Network Contract Direct Enhanced Service (DES).
They will provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCN’s practices and to help in tackling inequalities.
They will provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services. Through structured medication reviews (SMRs), they will support patients to take their medications to get the best from them, reduce waste, and promote self-care.
They will have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
They will develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.
You will also be responsible for working alongside our GP Pharmacy Clinical Lead to develop and implement an updated clinical strategy for our service. Additionally, you will lead on several key performance and operational criteria within the Pharmacy Service and provide support and leadership clinically across other PCN services as necessary.
The Clinical Pharmacist Service Manager is responsible for overseeing and optimising the delivery of pharmaceutical care within a primary care setting. This role involves managing pharmacy staff, collaborating with healthcare providers, implementing clinical programs, and ensuring compliance with regulatory standards. You will play a crucial role in supporting our Member Practices, enhancing patient care, improving medication management, and promoting the effective use of pharmaceutical resources.
You will be responsible for working proactively with the other Service Managers, collaborating on the wider strategic vision of the PCN and supporting across service. Operational cover across our Management team is expected, which will include holiday and absence cover for our Enhanced Access service, which operational hours are 17:00 – 21:00 Monday to Friday and 8:30 – 17:30 on Saturdays.
This role will be based across all four Winchester City PCN member practice sites: The Friarsgate Practice (Weeke & Badger Farm sites), St Clements Surgery, and St Paul’s Surgery, as well as remote working opportunities.

Pay: Up to £62,500.00 per year
