Main duties of the job
Work as part of a multidisciplinary antimicrobialstewardship (AMS) team to deliver the AMS agenda. On a patient level, you willsupport clinicians in the safe prescribing of antimicrobials through ward rounds,responding to ad-hoc queries and proactive surveillance.
With the support of the AMS team, you will developexpert knowledge in antimicrobials and infection management such as thepharmacokinetics, pharmacodynamics, spectrum of activity, safety profile andmonitoring requirements of antimicrobials.
Optimise transfer of patient care, though timelycompletion of medicines reconciliation and communication with GPs and communitypharmacy teams.
Accurately and sensitively communicate medicinesrelated information to a variety of healthcare professionals and patientsincluding those with language difficulties, physical or mental disabilities, ina way that facilitates shared decision making.
Support OUH dispensaries, and liaise with externalagencies (Such as aseptic production services or homecare providers) to ensuresafe, timely and appropriate supply of medications to patients.
