Job summary
Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking to recruit a full-time Band 4 Associate Practitioner for the North & West Hampshire Community Learning Disability Service.
This is an exciting time to join a busy and well-established multi-disciplinary health team, supporting the health care needs of people with a learning disability throughout the Locality. You will play a vital role as part of the community nurse team in a forward thinking and innovative team. You will take part in nurse meetings, well established group supervision and opportunities for professional development including participating in local and divisional quality improvement initiatives.
We are looking for a person to support the work of the registered practitioners in the community learning disability team. You will need to have experience of working with people with a learning disability and ideally educated to Foundation Degree level, or equivalent. You will have the opportunity to develop in the role and gain further skills, knowledge and competencies.
This post will give you the opportunity to contribute towards the nursing team and wider multi-disciplinary team. You will need to demonstrate effective leadership and inter-personal skills, good communication, be organised and self-motivated.
We see supervision and continuing professional development as important aspects of maintaining a quality health service for people with a learning disability.
Main duties, tasks & skills required
The Associate Practitioner will work as an active member of the Multi-Disciplinary Health Team in providing a high quality, efficient and effective service to service users.
The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner; predominantly nursing. They will work within relevant legal, ethical and competency and clinical frameworks in accordance with Southern Health protocols and policies.
The Associate Practitioner will be proficient, knowledgeable and competent in carrying out duties delegated to them by registered professionals within the multi-disciplinary team. Educated to Foundation Degree level, or equivalent, they will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of the service users, the service and the team.

* Foundation Degree in Health & Social Care or equivalent
* Full Driving licence.
* Able to attend further training and education in line with Trust Policy and Staff Competency Framework.
