An Excellent opportunity to join our Looked After Children Team within the Anglesey and Gwynedd Area. We are a small team dedicated to working with children, families and carers and supporting every opportunity to maintain their health, development an emotional wellbeing.
Main duties of the job
The post holder will work alongside the Looked After Children's (LAC) Nursing Team and lead working in a dedicated area within the Anglesey and Gwynedd area of North Wales, and will be based within a community health setting. The post holder will have professional responsibility to coordinate timely completion and implementation of high quality statutory health assessments and care plans for Looked After Children. The aim and purpose of the post is to provide services in partnership with multi-disciplinary teams including School Nursing, Health Visiting, Community Paediatricians and other LAC Nurses within and outside of Betsi Cadwaladr University Health Board to ensure the best possible health and social outcomes for the child/young person in the care of the Local Authority or on the Edge of Care. The post holder will be an advocate for LAC Children and Care Leavers.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
