Job summary We are recruiting for aMacmillan Personalised Care Support Worker who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients. This is an exciting opportunity to join the cancer outpatient team at Lister Hospital, in a brand new role. You will work closely with cancer patients to identify social and support needs through the completion of electronic holistic needs assessments (eHNA). Using the eHNA tool to identify any areas of concern for the patient and signpost as needed, facilitating personalisation of patient care. You will also assist with end of treatment summaries (EOTS) utilising the Macmillan My Care Plan eHNA system to improve patient experience. The role will include liaising with community and social services and working with the existing community link workers to ensure a seamless service across the cancer centre, particularly for vulnerable patients. As part of the multi -disciplinary team (MDT) you will contribute to service provision by providing elements of direct and indirect patient care and clerical work. You will be expected to facilitate and support care delivery, information giving and communication and coordination. You will also have the additional benefits of being a Macmillan post holder.
