Our Public Sector Client based in South Tyneside requires a Hospital Discharge Co-Ordinator/Administrator to join their busy Integrated Hospital Discharge Team. The role is office based and is running through to the end of March 2025. The hours of work are usually Monday to Friday with some flexibility on start and finish times. There may be occasions where some shift work is required over Bank Holidays and weekends on a rota basis. The hourly rate for this role is £13.69 per hour.
Overall Objectives of the Post:
Our Adults and Integrated Care Service is responsible for a range of services and activities that support people with social care and health needs, including carers. The approach used by the service puts more focus on supporting people to remain independent and to live better lives. Key Tasks of the Post:
1. Be responsible for the coordination and management of a daily position statement and circulate it to all partners.
2. Use active listening skills, observation, and communication to build relationships with all teams, partners, and providers.
3. Act as an ambassador for South Tyneside Social Services, promoting good working relationships in line with South Tyneside's PROUD values.
4. Confidently and professionally challenge decisions, delays, and queries impacting the flow of hospital discharge.
5. Participate in effective solution-focused discussions to manage flow, escalating to the Hospital Discharge Manager where there are significant delays or risks.
6. Ensure access to care and support on the day of discharge and work with partners to reduce delays and promote wellbeing.
7. Work closely with health, housing, and voluntary sector colleagues to ensure effective resource use across the system.
8. Work in accordance with safeguarding policies, supporting individuals in making choices and having control.
9. Develop awareness of services available at home and understand the roles and accountabilities of other agencies and professionals.
10. Oversee and manage daily tasks, phone calls, and email enquiries received into the Hospital Discharge Team.
11. Manage daily operational queries, maintaining a customer-focused approach.
12. Maintain resilience while working under pressure in a fast-paced service.
13. Work autonomously as well as within a team.
14. Adapt to change as the service demands.
15. Ensure compliance with the Council's data quality management arrangements.
16. Support with the management of data quality assurance as directed by the Hospital Discharge Manager.
17. Prepare and present reports of a statutory and non-statutory nature.
18. Engage in peer support and challenge.
19. Champion a commitment to equality and diversity throughout the service.
20. Identify own learning and development needs to improve practice and support CPD.
21. Contribute to the ongoing improvement and development of Adults and Integrated Care.
