Responsibilities
Conduct thorough assessments of clients' housing situations to identify needs and risk factors.
Develop and implement individualized action plans to prevent homelessness.
Provide ongoing support and guidance to clients throughout the housing assessment process.
Collaborate with local agencies and community organizations to facilitate access to resources and services.
Monitor client progress and adjust interventions as necessary to ensure effectiveness.
Gather and analyze data on housing trends and client needs to inform service improvements.
Maintain accurate and timely case records and documentation in compliance with organizational standards.
Requirements
Bachelor's degree in social work, public policy, or a related field.
Prior experience in housing services, social work, or case management is preferred.
Strong understanding of housing policies, programs, and resources available to clients.
Excellent communication and interpersonal skills, with the ability to engage effectively with diverse populations.
Ability to work independently and manage multiple cases simultaneously.
Strong analytical skills and proficiency in data collection and reporting.
