Job summary
The post holder will participate in the provision and development of the clinical pharmacy service to all wards in the Trust in accordance with current Medicines Management Procedures and will work on a rotational basis. There will also be a responsibility for working in the dispensary.
Main duties of the job
This role is hrs per week plus on call, weekends, Bank holidays and late shifts
In accordance with the current Trust objectives, the post holder will be responsible for ensuring that medicines reconciliations, on the assigned wards, are completed as soon as possible after admission. They will also be responsible for ensuring discharge prescriptions are dealt with quickly and efficiently, responding to medicines information queriesand conducting audits
The clinical pharmacist will rotate through a number of specialities every six months. These specialities will include medicine, stroke, trauma and orthopaedics, women and children, surgery, community and out patients.

Job responsibilities
As a clinical pharmacist working on the wards, you will demonstrate the ability to provide safe, clinically effective and cost efficient use of medicines to in patients.
8. To provide a routine clinical pharmacy services to the wards, including prescription monitoring
9. To provide pharmaceutical information and advice to medical and nursing staff and patients on the wards
10. To demonstrate a good core clinical knowledge.
11. To enhance and monitor the quality of patient care
12. To demonstrate a whole-system patient-focused approach.
13. To demonstrate good level of clinical reasoning and judgement.
14. To liaise with other clinical pharmacists to ensure efficient transfer of medicine related care.
15. To counsel patients on their medication and answer any other medicine related enquiry.
16. To ensure the personal quality and accuracy of work is maintained despite frequent interruptions
