Job summary Harlow South Primary Care Network has an excellent opportunity for a care coordinator join to our team, based at the Hamilton Practice in Harlow. The role is offered on a full time basis, for 37.5 hours per week. Main duties of the job Maintaining accurate and confidential patient records and documentation. Working closely with the data quality team to ensure the accuracy, completeness, and consistency of patient data. Assisting in ensuring compliance with the Quality and Outcomes Framework (QOF), Primary Care Network Investment and Impact Fund (PCN IIF), and other local and national standards. Participating in regular audits and data quality checks to identify and resolve discrepancies. Supporting the preparation and submission of monthly, quarterly, and annual claims and reports. Co-ordinating and minuting MDT and weekly care home meetings. Inviting and booking reviews for serious mental illness, learning disability, long term conditions and frailty. Support patients self-referring and inviting and booking carer reviews. Providing signposting for carers, e.g., to the social prescribing team. Taking initiative, identifying areas for improvement in patient care and data management processes, implementing solutions. Ensuring patients receive coordinated and comprehensive care while maintaining high data quality standards, ultimately improving overall health outcomes.
