Integrated pathways are looking for an enthusiastic and passionate candidate to join the department of Older People's Services at Ipswich Hospital, part of East Suffolk and North East Essex foundation nurse consultant. The post holder will be an experienced advanced level practitioner with a non-medical prescribing qualification, autonomous, expert skills in assessment, examination and providing diagnosis and management for patients across acute and community settings.

The integrated pathways division has the older people's services, therapies and community services for Ipswich and East Suffolk sitting within the same division allowing acute geriatric services to work seamlessly with community services and allows the flexibility to meet patient needs wherever they may be.
Main duties of the job
You will join a team of skilled frailty interface practitioners including nurses, therapists and Geriatricians. These services reach into the front door emergency department assessing our frail patients at the point of need and helping to streamline their care. Assessment will also take place in the frailty assessment base where a multidisciplinary team can provide acute and complex care. There will be an integral link with our reactive emergency assessment community team working towards conveyance and admission avoidance.
You will practice in conjunction with the multidisciplinary team and play an integral role in leading service development. The nurse consultant would manage the full frailty team as well as the community hospitals ACPs and ensure continuous development of clinical practice, research and standard of care. The development of the integrated frailty service will include developing policy, guidelines and pathways to ensure older and frail people have seamless access to the right care in the right place at the right time.
