Job summary To act as the single point of administrative and coordination support for the Older Persons Integrated Care (OPIC) Team within the Barnstaple Alliance Primary Care Network. This role, alongside the OPIC Team clinicians, will work closely with the member practices to provide care to our care home and housebound patients registered within the PCN. Hours available are 22.5hrs a week, worked over 4 or 5 days. Interviews planned for 23rd April 2025. Main duties of the job The OPIC Team consists of Nurse Practitioners and a GP Clinical Lead. The PCN Care Coordinator role is pivotal to ensuring all patients receive the best possible care and service through coordinating and supporting the delivery of all key activities of the OPIC Team clinicians. The role will involve coordination of home visits for housebound and care home patients. The role will also include supporting and coordinating focused work around the delivery of theQuality and Outcomes Framework (QOF) and specifications within the Network ContractDirected Enhanced Service (DES), including Enhanced Health in Care Homes (EHCH) and Structured Medication Reviews (SMRs).
