This is a part-time Community Nursing position which is designed to contribute to avoiding hospital admission and maintaining patients' independence in their own homes throughout the Waterlooville locality. This is achieved by both routine visits and responding to Urgent Crisis Response calls, which are a key aspect of the role going forward. The UCR process involves a team working alongside SCAS, offering acute visits with other members of the MDT, and using a community-based skill set to try to reduce the need for an ambulance to attend and prevent patient admission.
This post offers the opportunity to work closely with all members of the ICT in primary, secondary care, and adult services, as well as working closely with care home staff.
The successful candidate would be required to actively participate in ICT/MDT meetings along with other team members and the wider MDT disciplines, coordinating a plan of care and working autonomously within a registered practitioner's scope of practice. They will be expected to be involved in care planning and identification of person-centered care delivered in the home setting. Ideally, the successful candidate will possess excellent communication skills and be competent in working together with the larger multidisciplinary team.
The job involves a proportionate amount of time driving; therefore, you will need to be a car owner and have access to a vehicle for work with the correct level of insurance.
Main duties of the job
Working within professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy, or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post-holder will manage, coordinate, and delegate work, ensuring referrals are dealt with according to clinical need. Holistic assessments and care plans are recorded in line with Trust and professional record-keeping standards, and clinical care always meets acceptable standards of patient safety and quality outcomes.
The post-holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post-holder will also have a responsibility for re-enablement, rehabilitation, health protection, and promotion programmes that improve and reduce inequalities.

* BSc Nursing or RN currently registered with NMC.
* Practice assessor/supervisor.
* Post-registration, specialist training relevant to specialty.
