1. Set up and manage sustainable Healthy Communities projects/programmes that deliver measurable community impact, focusing on tackling health inequalities.
2. Complete monitoring and evaluation tasks as requested by your line manager, adhering to current data protection protocols.
3. Deliver specific Healthy Community projects/programmes (e.g., Exercise Referral, gym-based classes, falls prevention) based on your training/qualifications and discussions with your line manager.

1. Qualified instructor in relevant discipline to deliver sessions independently (e.g., Gym Level 3, NGB Level 2 or higher).
2. Experience in planning, delivering, and monitoring sessions.
3. A passion for improving the health of the local community.
4. Proficiency in MS tools.
