The role will focus around 1-1 care of a patient with complex health needs within their home setting. Close working relationships with the patient, their family and the team to achieve and maintain this high level of care are essential.

The Care of Patient at home team provide complex care to an individual patient within their own home and social settings on a 1-1 basis. You will be working closely as part of the Care of patient at home team and the patients family to ensure all health and social care needs are met within the patient’s own environment including day center.
The role would be to cover two 4 hour weekend shifts every other Saturday and Sunday
An understanding of system one would be desirable but not essential as full training would be given.
Due to the nature of the role, A set of advanced skills are required within this role and full support and training will be in place to ensure you are fully competent and confident within the role.
Daily duties include:-
Chest physio
Tracheal suctioning
Tracheal Care
Cough assist
Personal cares including showering
Administration of medication via feeding tube
Setting up of enteral feeding and management throughout the day.
Enteral feeding tube care
Monitoring and ensuring the management of the patients equipment including wheelchair and hospital equipment.
Please note although list is extensive full training will be offered to support within gaining competency of all areas. The provision of extra bank shifts may be available and alternate skill sets my be required for other shifts.

clinical duties:
• To deliver direct and indirect patient care.
• Undertakes delegated care without direct supervision where appropriate
• Participates in the ongoing support of clients and their carers / relatives
• Observe a personal duty of care when using equipment, ensuring own safety and the of other
• Work in a collaborative manner with other healthcare professionals and agencies to deliver a high standard of care
• Be aware of the health and safety issues regarding the handling/exposure to bodily fluids when caring for patients
• Contributes to the promotion of Health and well being, encouraging people to view their health as a positive aspect of their lives, through planned activities and shared decision making.
• Supporting the patient to attend activities at the day center whilst ensuring health needs are met during this time.
• Assisting to travel to and from home (please note specialist transport is in place however there will be a requirement to travel alongside the patient as part of your role).
• Record keeping via patient medical record of care and interventions performed – desirable skill set around System one however full training will provided.
Professional Duties:
• Impart knowledge plans and instructions clearly and effectively
• Establish clear lines of communication at all levels
• Communicate relevant and factual information to patients and carers, using sensitivity and tact
• Contributes to the understanding of patients where there are communication difficulties.
• Maintain patient and professional confidentiality at all times.
• Report any change to patients condition to the appropriate member of the MDT at earliest opportunity.
• Attend and contribute to meetings held by the team and the Trust.
Managing Information:
• Use appropriate information to make timely and effective decisions .
• Contribute to the development of information systems.
• Obtains transmits and stores information relating to the delivery of care services (this may involve auditing)
• Participate in the use of information and technology including inputting data on the computer system enabling other members of the team to use.
• Provide information in a form that meets the understanding of the receiver at all levels.
• Use of oral, written and computerized information that facilitates continuity of patient care throughout the 24 hour period.
• Completes documentation in accordance with Trust’s policies and procedures.
Managerial Duties:
• There are no managerial duties required in this post.

Managing Self
• Participate in regular supervision.
• Attend all mandatory training.
• Participate annually identifying, developing and agreeing your own development plan
with your Line Manager using the Trust Appraisal.
• Comply with all Trust policies, procedures and protocols.
• Carry out duties with due regard to the Trust’s Equal Opportunity Policy.
• Seek advice and support from Line Manager whenever necessary.
• Maintain professional conduct including appearance at all times.
• Ensure maintenance of Professional Registration.
