Job summary Admission Avoidance Team We are seeking to appoint a dynamic, confident, innovative and flexible Nurse or AHP with excellent communication and organisational skills. The post is 37.5hours per week and can be offered as either a permanent position or secondment opportunity. This post holder will join our existing team of high functioning, highly motivated Advanced Clinical Practitioners, who deliver a wide variety of services including 2 hour Urgent Community Response, Hospital at Home/Virtual Ward, Falls pick up services and dedicated Care Home support, all enable people to be cared for in their usual place of residence. The Advanced Clinical Practitioners operate seven days a week between 08.00 to 20.00. The post holder will work with partners system wide to provide integrated care across the health economy. The applicant will have qualified and hold an MSC in Advanced Clinical Practice.
