Rehabilitation Assistant, Outpatients and Rehabilitation
* Work as a member of the multi-professional team to provide holistic care to adult patients, their carers, and families.
* Support the Rehabilitation team in providing clinical, administrative, and health & wellbeing support to patients, carers, and families, including running groups (with support) and in the inpatient unit.
* Competently work alongside patients and their families with support and guidance from a Physiotherapist to maximize mobility, independence, and a sense of wellbeing, contributing as part of the multi-disciplinary team to manage pain, breathlessness, fatigue, and other symptoms.
* Work effectively together and build relationships with the existing MDT. Promote the Rennie Grove Peace Hospice Care philosophy within the local community.
Main duties of the job
* Work within the rehabilitation therapy team in the planning and delivery of a palliative rehabilitation model of service to ensure patients have access to interventions enabling them to reach their fullest physical, psychological, social, and vocational potential consistent with their limitations, desires, and life plans.
* Support the physiotherapist in the writing and reviews of individualized care plans as required. Maintain succinct clear records on the electronic patient records.
* Plan, prioritize, and manage own workload on a day-to-day basis combining a range of IPU patient and outpatient related activities as delegated by the physiotherapist.
* Implement exercise programmes as set by the physiotherapist and provide advice in relation to progress, including exercise group provision working with volunteers.

Job responsibilities
* Support the rehabilitation team with education and training of members of the multi-disciplinary team and community services. Attend relevant training courses and meetings, especially those relating to self-management/rehabilitation in palliative care.
* Participate in the annual appraisal programme and adhere to the Hospice policies and guidelines. Ensure all feedback, evaluation, and outcome measures are followed and completed appropriately.
* Develop relationships with relevant members of the multi-disciplinary community team.
* Attend MDT/report meetings as required.
* Set up and prepare room for exercise groups.
* Assist with monitoring and checking of equipment used within the hospice and report any issues.
* Act as a Rennie Grove Peace ambassador within the community.
Person Specification
Qualifications
* NVQ qualified to Level 3 or equivalent or Level 4 or rehab/physiotherapy/occupational therapy assistant qualification/experience.
* Experience of working in a related care setting.
* Experience of working as either a health care assistant, physiotherapy assistant, occupational therapy assistant, or running exercise groups.
* Understanding of palliative care and Hospice philosophy.
* A good understanding of technology with a working knowledge of Microsoft Office packages (i.e., Word, Excel, and Outlook).
* Able to build good working relationships.
* Able to communicate effectively both verbally and in writing.
* Good administrative and record-keeping skills with attention to detail.
* Strong organizational skills.
* Sensitivity and understanding of the needs of the volunteers and customers, many of whom may have experienced Rennie Grove Peace services or be bereaved themselves.
