The post holder will be part of the Reablement Service – In-house provider services. Islington’s Reablement service provides a short intervention for Islington residents of up to six weeks with a view to supporting people to become more independent. The Reablement Social Worker will be part of a multi-disciplinary team comprised of team leaders, home support workers and case managers, and working together with Pharmacists and Therapists carrying out targeted work to improve the independence of people going through a period of Reablement in Islington. The role of the Reablement Social worker will be to guide people with more complex needs through the up to six week intervention. They will take a lead role in drawing on the expertise of team members to coordinate a person’s Reablement journey with a view to maximising independence and working to meet Reablement goals. The role will include completing social work specific activities and assessments in line with Safeguarding Adults procedures and the Mental Capacity Act. The post holder will report to the Reablement Manager on a day to day basis for all operational activities and receive formal supervision from the Senior Practitioner Social worker. Flexible working over a 7 day working week will be required to fit in with the needs of the service: • To work in a multi-agency context, sharing information, joint assessments and care planning to provide a holistic understanding of need and provision of services. • To take responsibility for promoting and safeguarding the welfare of vulnerable adults in your care and those who you come into contact with. • To carry a caseload of varied range of complexity. • To take on care coordination responsibility for their caseload. • To undertake Mental Capacity Assessments in accordance with the Mental Capacity Act 2005 • To ensure that the Care Act 2014 is implemented in all aspects of their role. • To work with service users, carers and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible. • To obtain informed consent for all care coordinated cases. • To ensure that their Team Manager is kept informed of any significant events relating to the service. • To provide advice and consultancy to colleagues within the immediate team as well and to a wider range of stakeholders, which may include the NHS, the Independent sector and members of the public as appropriate. • To keep records of work undertaken and entering data on the database in accordance with Council Policy. • To attend and contribute to multi- disciplinary and other meetings as required. • To meet contractual agreement by attending regular supervision. • To use and assist others in the use of information technology systems to carry out duties in the most efficient and effective manner. • To achieve agreed service outcomes and outputs, and personal appraisal targets, as agreed by the line manager. • To undertake training and constructively take part in meetings, supervision, seminars and other events designed to improve communication and assist with the effective development of the post and post holder. • To carry out duties and responsibilities in accordance with the Council’s commitment to customer service excellence and ensure compliance with the customer care standards. • To be committed to the Council’s core values of public service, quality, equality and empowerment and to demonstrate this commitment in the way duties are carried out. • To ensure that duties are undertaken with due regard and compliance with the Data Protection Act and other legislation. • To carry out duties and responsibilities in accordance with the Council’s Health and Safety Policy and relevant Health and Safety legislation. • To follow the Pan London Safeguarding Adults procedures to investigate and manage risk where individuals have experienced or are likely to experience significant harm. To provide written and verbal reports which are concise, informative and based on analysis of complex evidence which can be used within legal proceedings. • To use a variety of social work methods to remedy personal and social difficulties experienced by specified clients, including care, support, counselling, supervision, protection and other therapeutic methods. Rate will be £32 umbrella (this will be applied at placement stage
