An exciting opportunity has arisen for an experienced practitioner to join the Division of Medicine here at West Hertfordshire NHS Teaching Trust.
We are looking for a highly skilled, motivated, and dynamic practitioner to complement and enhance our team as the Stroke Specialist Practitioner who wishes to work towards their Advanced Clinical Practitioner qualification in the future.
The successful candidate will be innovative and someone who strives to deliver person-centered care. The post holder will need to work autonomously and be confident in their own decision making, utilising their knowledge and skills to deliver excellent care. The post holder will be required to demonstrate the ability to prioritise workloads and use their own initiative.
The successful candidate will provide specialised clinical expertise, expert advice and leadership to patients, careers, trust and trust staff with a particular focus on our patients within Stroke services.
The post holder will act as a practitioner and a source of expert advice.
The post holder will be responsible in improving the care and services for patients with Stroke.
They will enable and facilitate change to deliver a modernised service promoting innovative ways of working that are patient centered using quality improvement methodology.
The post holder will be a visible, pro-active and accessible leader, acting as a clinical resource and ensuring the sustained delivery of high standards of care.
The post holder will provide education in Stroke care to improve patient experience and continuity of care.
The post holder will be an integral part of the nurse-led stroke video triage pilot alongside EEAST.

The post holder will be professionally accountable and maintain the standards of the professional practice as set by their professional body.
They will be responsible for ensuring safe and effective practice, enhancing patient experience, undertaking clinical audit, supporting research, monitor and improve standards of care, work alongside the multidisciplinary team and be responsible for efficient and effective use of resources.
The post holder will be part time 23 hours per work; with the aim of providing a seven service alongside the team.
