We are seeking a dedicated and experienced Senior Occupational Therapist to join the team at Wrightington Wigan and Leigh NHS Trust. Role and Responsibilities:
* Deliver high:standard assessments and rehabilitation interventions to patients in an intermediate care setting.
* Develop and implement individualized treatment plans, focusing on maximizing patient independence and safety.
* Work collaboratively with a multidisciplinary team, including nurses, social workers, and other allied health professionals, to provide comprehensive care.
* Manage a diverse caseload, ensuring timely and effective care delivery for community:based and inpatient services.
* Educate and support patients and their families in understanding treatment plans and rehabilitation goals.
* Actively contribute to the development and review of clinical protocols and best practices.
Requirements:
* Diploma/Degree in Occupational Therapy or Physiotherapy.
* HCPC Registration.
* Experience in intermediate care or rehabilitation settings.
* Ability to effectively manage care for frail and elderly patients.
* Strong community experience and a proactive approach to patient care.
* Excellent communication and interpersonal skills.
Skills:
* Strong clinical skills in assessing and treating a range of conditions.
* Ability to detect the deteriorating patient.
* Ability to risk assess patient assessment, treatment, and discharge.
* Ability to assess and treat motor and sensory skills as well as physical functioning.
* Ability to assess and treat cognitive and affective impairment and its impact on function and the patients ability to maintain their health and safety.
* Ability to structure holistic assessment and the OT process within a framework that provides clear clinical reasoning for recommendations made in relation to a patients needs and care pathway.
* Evidence of CPD.
