If so, we have a vacancy for a CHC (Continuing Healthcare) led Discharge to Assess (D2A) Clinical Case Manager within our All Age Continuing Care (AACC) team.
As a D2A Case Manager, you will be responsible for the assessment and review of care needs of patients under the CHC led D2A pathway.

This post requires you to have excellent verbal and written communication skills. The team has strong links with community services, and collaborative working within the multi-disciplinary team and with our community colleagues in health and social care is essential.

This is an excellent opportunity to gain a wide range of experience working with individuals who have a broad range of health care needs and to work in a service that is constantly committed to improving patient care and experience through discharge planning, working in collaboration with the hospital teams, and assessment for CHC eligibility.
Once a patient is assessed as being eligible for NHS Continuing Healthcare (CHC) D2A pathway, or for CHC Fast Track pathway, our role is to liaise with the patient, family, and hospital staff and start gathering information regarding the patient’s care needs, and feed back to the relevant AACC clinician. You will be responsible for arranging attendance at the assessment for eligibility to NHS Continuing Healthcare funding.
Using your existing and new knowledge and clinical skills, you will ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care.
You will work closely with the AACC teams, hospitals, community services, and care homes to maintain and improve systems and processes to facilitate a smooth referral, assessment, and review process to support patients. You will escalate complex issues to senior managers for timely query resolution.
Our team’s office is based in Epsom, Surrey, with agile working conditions, meaning a combination of working from the office, patient’s residence, and your home. Visits to the hospital are also required sometimes to see the patients and gather clinical information before they are discharged. Ideally, you will live within the Surrey Heartlands geographical area, as you will be expected to travel across Surrey for assessments and travel to your induction and meetings as required by the service.
