To use clinical expertise to analyse and challenge health information and assessments to ensure that an accurate assessment of childrens needs are documented. To support implementation of the personal health budget process. To communicate effectively with colleagues, partner organisations, service users and their representatives. To maintain a rapid and effective response to requests for advice and assessments. To work within the timescales outlined in the Childrens Continuing Care Policy. To work with colleagues to implement the care pathway for discharge of Leeds children with complex needs. To meet statutory duties under the SEND Code of Practice, engaging in the Education, Health, and Care planning process to ensure alignment of EHCP and Continuing Care. You will be expected to demonstrate knowledge and understanding of all aspects of safeguarding children and work within organisational policies. Handle information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or co-ordinating such information in accordance with the Data Protection Act. Make judgements on a range of issues relating to childrens continuing care and personal health budgets, using own expertise and within responsibility level. Plan and manage own workload to weekly, monthly and annual timescales to ensure continuity of service. Plan and co-ordinate meetings as appropriate. Provide day to day supervision of support staff ensuring all policies are adhered to and agreed objectives are monitored. Attend regular supervision and demonstrate development through the supervision process. Communication Skills The post holder will be expected to communicate with a wide range of people, at all levels from different organisations, as well as families and carers and those who have an interest in Children's and Young Peoples continuing care. Communication will include verbal and written, as well as presentations, education, and training.. a) The post holder will be expected to be able to communicate in situations which require highly developed communication skills, and with people on complex and sensitive issues relating to clients needs, provision, and future planning. b) To promote interagency / partnership working to improve service delivery. c) To work in partnership with other agencies to provide a quality service for children. d) To ensure that parents / carers / childs views are taken into account, to ensure that parents / carers / children are fully involved in the review and planning process. e) To work closely with families and clients to ensure that they are prominent in the decision-making process. f) To work with families to resolve problems which may arise. g) To have the ability to work with individuals at all levels. h) To encourage partnership working with other agencies. Safeguarding Children Under section 11 of the Children Act 2004 all NHS staff must ensure that their functions are discharged with regard to the need to safeguard and promote the welfare of children (Working Together to Safeguard Children 2018). All staff need to ensure, as part of their work with children and families and with adults who are parents or carers who are experiencing personal problems, that the needs of the children are considered and that where necessary they are assessed and appropriate referrals are made.
