Main duties of the job
* Working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need.
* Delivering a high quality clinical service to service users and their families.
* Managing a complex caseload and responsible for clinical and caseload supervision of other team members where identified.
* Operating as care coordinator for a defined case load of service users.
* Delivering therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.

Job description
Job responsibilities
You will be responsible for working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need. You will be responsible for care coordination of an identified caseload.
You will be responsible for undertaking clinical assessments, in accordance with the clinical pathway, in order to establish the person's needs, goals, risks and strengths in order to generate a formulation based care plan agreed with all involved. You will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate.
Working Monday to Friday 9am 5pm and requires travel across a large geographical area, access to suitable transport is required.

Qualifications
Essential
* Registered mental health professional E.g. Nurse or Occupational Therapist or Social Worker
* Post registration course/s at degree or diploma level
* Practice supervisor / Assessor (Nursing candidates only)
* Active on NMC/HCPC register
Desirable
* Degree in Nursing / OT/ Social Work or Equivalent
* Psychological interventions qualification (CBT, EMDR, CAT, DBT, Family Therapy)
* Non-Medical Prescriber
Experience
Essential
* Evidence of extensive post registration experience of working with adults experiencing mental health related difficulties OR experience of working with people who have difficulty in engaging with services and have complex mental health needs
* Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting
* Experience of integrating care delivery across multi-disciplinary/multi agency teams
* Experience of contributing to CPA process
Desirable
* Experience of leading CPA process
* Experience of working in community setting and making decisions autonomously
* Experience of carrying out clinical audit
