Job summary We are 2 practices based across 2 sites with circa 33,000 registered patients across the north of Portsmouth City. We are looking to recruit an Admin Care Coordinator on a Wednesday and Friday, to join our extensive practice and PCN teams. Our multi-disciplinary team already includes GPs, Nurse Practitioners, Nurses, Healthcare Support Workers, Phlebotomists, Pharmacists, Pharmacy Technicians, Social Prescribers and Physiotherapists. We have excellent communication and team-working. Main duties of the job The Admin Care Coordinator will work with the PCN central team supporting the delivery of NHS Englands Comprehensive model of Personalised Care and the Early Cancer Diagnosis Direct Enhanced Service. This pivotal role will support multidisciplinary teams by helping to coordinate bi-weekly meetings, and also support the wider PCN team by assisting with general administrative duties that will help promote a positive patient journey.
