Bradford District Care NHS Foundation Trust (BDCFT) is committed to delivering rehabilitation services to reduce health inequalities and improve the health and social care outcomes for individuals in our communities with Severe Mental Illness (SMI). The post holder will lead the team within mental health rehabilitation services to support and develop the team to focus on rehabilitation. They will also work with the flow team in delivering clinical processes that identify, review and engage those service users stuck within acute inpatient beds, cycles of readmission, or inappropriate residential placements.
The post holder will provide clinical and team leadership, displaying sound reasoning to support and enable timely step down into a variety of settings including NHS inpatient and community rehab provision, supported housing/living, or more independent community settings.
This is a specialised clinical post, and the post holder will use advanced caseload management and clinical skills, to support the development of designated rehabilitation services over a variety of settings as required by clinical managers that deliver evidence-based interventions to their caseload stepping provision up and down as required, whilst recognising existing good practice and opportunities for wider service developments.
Working closely with a variety of professional teams at place, Clinical Leads and the Rehabilitation Programme Manager, the post holder will be proactive in developing, embedding and evaluating sustainable rehabilitation pathways and environments to meet NG 181 and GIRFT recommendations.
Key Responsibilities:
* To ensure the safety needs of the service user are met and take appropriate action when risk is identified.
* To provide highly specialist assessment of clients with severe mental illness and complex rehabilitation needs based on the appropriate use, interpretation, and integration of data from a variety of sources.
* To line manage as delegated a multi-disciplinary team, take responsibility for operational management processes, such as recruitment, CPD, and appraisals.
* Ensure adherence to trust policies and standard operating procedures.
* To formulate plans for highly specialist intervention relating to any identified rehabilitation needs whilst supporting the overall wellbeing and risk.
* To provide specialist advice, supervision, case discussion & formulation to NHS, social care providers and third sector staff and carers to support assessment of rehabilitation and transition needs in the most appropriate setting.
* To provide leadership in mental health rehabilitation teams.
* To support robust risk assessment and management offering specialist positive behavioural support plans where appropriate.
