Job overview
To work as a Band 6 Community Mental Health Practitioner in the Dementia Intensive Support Service in Mid Essex. The post holder will be responsible for providing rapid assessments for people living with dementia or suspected dementia in crisis situations that require home treatment to avoid hospital admission.
The successful candidate will be trustworthy, positive about change, and passionate about the services we provide. You will work independently and as part of a team, requiring excellent verbal and written communication skills.
Main duties of the job
* Provide high quality, evidence-based, holistic care to patients.
* Assess, plan, implement, and evaluate packages of care based on individual service user needs.
* Supervise and manage junior team members.

* Knowledge of dementia and understanding of frailty in older adults.
* Experience in performance management and supervision of lower banded staff.
* Current UK driving license and access to a car for work purposes.
Person specification
Education
* Registered Mental Health Nurse or Registered General Nurse.
* Good standard of written and spoken English.
* Postgraduate qualification or commitment to work towards this.
Skills
* Good oral communication skills.
* Ability to manage and coordinate multi-disciplinary meetings.
* Experience of working in the Mental Health environment.
Knowledge
* Understanding of health legislation and principles of CPA and Care Management.
* Knowledge of current best practices in mental health nursing.
Personal Qualities
* Current UK driving licence and access to a car for work purposes.
